Serpentine belt issues, shredding belt, chirping or squeaky

I have a 95 transport with the 3800 engine.  The. Belt is noisy and it chirps loudly. I. Replaced the belt, but it shredded the outside of the belt off. Then I had an idler pulley failure. Replaced the pulley but then belt shredded. Further, and still have the chirp. What is the likely cause?

The wear on the belt indicates that there is a pulley alignment problem.

Take the belt off and try to visualize each pulley to the crankshaft pulley. There is probably a loose or bent bracket that can usually be fixed by shimming the bracket on one of the connecting bolts to align the pulley properly if needed.

Thank you for the response. I think I found the problem.  I repositioned the ac bypass pulley and replaced the belt. So far no chirping or squeaking.  Keeping my fingers crossed!
